That means you have the opportunity to build a strong relationship with your own family doctor based on trust, confidentiality and a high level of quality care. We also have a doctor on call if the need arises giving you peace of mind that medical help is always at hand.

As a Finley Medical Centre client, you also have the benefit of a number of visiting specialists including a Counsellor, Diabetic Educator, Visiting Surgeon, Visiting Gynaecologist, Women's Health Nurse, Podiatrist, Pathology Services and other allied healthcare workers so you do not have to worry about travelling long distances to access specialist care. And our strong link with Finley Hospital means we can take care of any hospital admission that you or a family member may need.

  • How do you know if you (or a loved one) are eligible for residential Care?

    If you are considering coming into one of Finely Regional Care aged care facilities, the first step is to be assessed by an Aged Care Assessment Service (ACAS).

  • What is an Aged Care Assessment Service (ACAS)?

    This service is provided for people over 65 years of age and is made up of geriatricians (doctors specialising in aged care), community nurses, social workers, occupational therapists, and physiotherapists. Together they provide these important services: - Specialist aged care assessment Information about community services and how to be referred to them. Referral to health professionals and specialist clinics. Information about respite care and how to arrange it.

  • How is the assessment carried out?

    The various professionals included in the ACAS look at and assess your particular needs which help them decide which level of care is best suited for you. They will then put their recommendations into an Aged Care Assessment Report (ACCR).

  • What is an Aged Care Assessment Report (ACAR)?

    An ACAR is a report which is compiled after you have been assessed. It outlines your particular care and personal needs and other relevant information and recommends the most appropriate level of care that you need. When you come to see us at Finely Regional Care about entering one of our aged care facilities, this report will give us vital information which will help us decide which facility is best suited to you.

  • How do you arrange an aged care assessment?

    You can ask your doctor, a social worker or a community nurse to arrange an aged care assessment, or you can contact the Department of Health and Ageing.   • How do I apply to Finely Regional Care for aged care?

    Simply phone us on 03 5883 4916 and we will send you an application kit which contains an application form for you to complete. It is a standard form, issued by the department of Health and Ageing. Once you have completed the application form we ask that you send it to us along with your Aged Care Assessment Report (ACAR) We will contact you once we have received your application and report.
